What You'll Do
- Provide care in SNF and long-term care settings
- Manage chronic conditions; perform full assessments
- Close care gaps (HEDIS, STAR measures)
- Lead care plans with facility teams (fast decisions)
- Educate and support patient families
- Document and track outcomes (modern tools)
Must-Haves
- Active, unrestricted APRN license in Kentucky
- Board certified; Medicare credentialed
- 6+ months SNF or long-term care experience
- Comfortable with EMR and team-based care
- Able to work onsite in Monticello, KY
Nice to Have
- Experience with HEDIS or STAR measures
- Multi-site rounding experience
