Conduct face-to-face hospice evaluations
Support attending physicians and IDG (interdisciplinary group)
Manage pain and symptom control plans
Document care progression and patient eligibility
Visit patients in homes or local facilities
Collaborate with hospice team to meet patient goals
Counsel families through end-of-life decisions
Completed accredited Nurse Practitioner Program
Active Illinois NP and RN licenses
Illinois Controlled Substance License + DEA registration
2+ years in hospice, palliative, oncology, or symptom management
Pay: $65/hr

Schedule flexibility for PRN or Part Time availability
Full Time, PRN, or Part Time • on-site in Chicago
Minimal travel within local patient radius
