Nurse Practitioner Opportunity
Join our wound care team as a Nurse Practitioner and provide essential care to patients in their homes or nursing facilities. You will assess, treat, and manage various wound types, working closely with patients, families, and healthcare teams to develop personalized treatment plans and ensure optimal healing. This role offers autonomy, flexibility, and the ability to make a significant difference in patients' lives.
Key Responsibilities:
Wound Care Management:
Assess, diagnose, and treat patients with acute or chronic wounds, including but not limited to diabetic ulcers, pressure ulcers, surgical wounds, and venous stasis ulcers.
Develop and implement individualized treatment plans for wound care, focusing on optimal healing and infection prevention.
Apply appropriate wound dressings, debride wounds as necessary, and manage related complications such as infections or pain.
Collaborate with the patient’s primary care team and specialists to ensure comprehensive care.
Patient Care:
Conduct home or facility visits to provide ongoing wound care and monitor the healing process.
Educate patients and their families on proper wound care techniques, preventive measures, and the importance of compliance with treatment plans.
Provide emotional support to patients and families, addressing any concerns related to wound care and healing progress.
Documentation & Reporting:
Maintain accurate and up-to-date documentation of patient assessments, care plans, progress notes, and treatment outcomes in the electronic medical records (EMR) system.
Prepare reports and summaries of patient care for medical and administrative teams, ensuring timely and thorough communication.
Coordination & Scheduling:
Coordinate visit schedules with the agency’s administrative team to ensure timely and efficient patient care.
Work collaboratively with the agency’s team to adjust schedules as needed and ensure proper continuity of care.
Provide updates to the agency regarding patient progress, needs, or any issues that require further attention.
Compliance & Best Practices:
Adhere to all regulatory, clinical, and ethical standards of care, including infection control practices, patient confidentiality (HIPAA), and safety protocols.
Stay current with the latest wound care treatments, evidence-based practices, and medical technologies to ensure the highest level of care.
Qualifications:
Education:
Master’s degree in Nursing (MSN) and board certification as a Nurse Practitioner (NP).
Wound care certification (e.g., CWCN, CWS) preferred, or willingness to pursue certification.
Licensure:
Active and unrestricted Nurse Practitioner license in the state(s) of practice.
Ability to travel and provide care across various locations, including patients’ homes and nursing facilities.
Skills & Abilities:
Strong knowledge of wound care principles, infection control, and evidence-based treatment modalities.
Excellent communication and interpersonal skills to interact with patients, families, and healthcare professionals.
Ability to manage time effectively, prioritize tasks, and work autonomously in a mobile, patient-focused environment.
Proficiency in using electronic medical records (EMR) systems for documentation and reporting.
Physical Requirements:
Ability to travel and visit patients in their homes or nursing facilities, which may include travel by car or other means.
Ability to perform physical tasks such as lifting, bending, and assisting patients during treatments.
