Job Title: Nurse Practitioner (Mobile Healthcare Provider) Location: Multiple Regions Across South Carolina Job Type: Full-Time / Part-Time / PRN
Position Summary:
We are seeking dedicated and compassionate Nurse Practitioners to join our growing mobile healthcare team serving communities throughout South Carolina. As a provider, you will deliver high-quality, patient-centered primary and preventive care to individuals in residential homes, assisted living facilities, group homes, and other non-traditional settings.
This is a mobile role, and candidates must be comfortable traveling within a 30–45-mile radius of their home location.
Key Responsibilities:
Conduct comprehensive physical assessments, diagnostic evaluations, and develop treatment plans.
Provide acute, chronic, and preventive care to a diverse patient population.
Collaborate with patients, families, facility staff, and other healthcare providers to ensure continuity of care.
Perform point-of-care testing and interpret results.
Maintain accurate and up-to-date patient records using electronic health systems.
Adhere to all applicable federal, state, and local laws and standards related to patient care and documentation.
Travel daily to patient homes, assisted living facilities, and similar environments.
Qualifications:
Active Nurse Practitioner license in the State of South Carolina.
No sanctions or disciplinary actions on current or past licenses.
National board certification (AANP or ANCC) in family, adult-gerontology, or related specialty.
Valid DEA license or eligibility to obtain one.
Current BLS certification.
Valid driver’s license, reliable transportation, and proof of auto insurance.
Ability to work independently in non-traditional healthcare settings.
Strong clinical, communication, and organizational skills.
Preferred Qualifications:
1+ years of clinical experience as a Nurse Practitioner, particularly in home health, geriatrics, or community-based care.
Familiarity with EMR systems and mobile documentation tools.
Work Environment & Travel:
Must be able to travel 30–45 miles from your home base to reach patients.
Mileage reimbursement provided.
