    Nursing Supervisor / Case Manager (RN)

    Location: North Branch, MN Company: St. Raphael’s Home Care Job Type: Full-Time | In-Person Salary: $90,000 – $100,000 annually Schedule: Monday – Friday | No weekends!

    St. Raphael’s Home Care is a nurse-owned, faith-based, and rapidly growing Catholic home care company located in North Branch, MN. We are dedicated to providing compassionate, high-quality care to our clients and are seeking a Nursing Supervisor / Case Manager (RN) to join our team.

    What You’ll Do

    As a Home Health RN, you will:

    Manage your own schedule and ensure client visits are completed promptly.

    Provide skilled nursing services, including:

    Medication setup and management

    Wound and catheter care

    Other skilled nursing tasks as needed

    Complete thorough, timely documentation.

    Serve as a clinical resource for field nurses.

    Onboard caregivers in compliance with Minnesota Statute 144A.

    Conduct client reassessments and assist with new admissions or change-in-condition visits.

    Assist field nurses with coverage as needed.

    Schedule client visits and maintain up-to-date schedules.

    Qualifications

    Current RN license (required)

    Home care experience: 1+ year (preferred)

    Ability to reliably commute or relocate to North Branch, MN

    Compassionate, organized, and detail-oriented
