Job Title: Nurse Practitioner - Primary Care
**Position Overview: **
We are seeking a compassionate and dedicated Nurse Practitioner (NP) to join our Primary Care team. **Key Responsibilities: **

- Develop and implement individualized treatment plans, including prescribing medications and therapies.
- Perform and interpret diagnostic tests, including laboratory and imaging studies.
- Provide patient education on disease prevention, health maintenance, and management of chronic conditions.
- Collaborate with interdisciplinary teams to ensure continuity of care and optimal patient outcomes.
- Maintain accurate and timely electronic medical records (EMR) for all patient interactions and treatment plans.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Stay current with best practices, continuing education, and advancements in healthcare delivery.
- Serve as a resource and mentor for nursing staff and students.
- Participate in community outreach programs to promote health education and disease prevention.
**Qualifications: **
- Master’s degree in nursing with a focus on Adult, Family, or Pediatric Primary Care.
- Current state licensure as a Nurse Practitioner (NP).
- National certification as a Nurse Practitioner from an accredited organization (e.g., AANP, ANCC).
- 1 years of experience in a primary care or similar clinical setting preferred.
- Strong clinical assessment skills and a comprehensive understanding of patient care protocols.
- Excellent communication skills, both verbal and written, to effectively interact with patients and team members.
- Ability to work independently as well as collaboratively in a fast-paced environment.
- Proficient in electronic medical record systems and other healthcare technology.
