    Job Description

    Reports to the Nursing Leader/Administration. The Nurse Coordinator is a Registered Nurse who under the direction of department leadership develops, implements, monitors and evaluates a program of care for patients. Coordinates care across the continuum to facilitate quality outcomes. Collaborates with other healthcare team members including physicians, nurses, extenders, ancillary and support staff to ensure optimum patient care and satisfaction. Provides education to patients and family regarding disease process. Trains and directs a nursing team in the optimal care of the patient.

    EDUCATION & EXPERIENCE REQUIRED:

    • Graduate from an accredited school of nursing
    • Minimum of 3-5 years of progressively responsible clinical nursing practice in vascular specialty
    • Bachelor's Degree in Nursing (preferred)
    • Certification in vascular specialty (preferred)
    • Current clinical knowledge and analytical abilities to formulate effective nursing care plans
    • Ability to perform a wide variety of professional nursing services

    SKILLS & COMPETENCIES REQUIRED:

    • Effective communication with patients, families, and medical personnel
    • Basic computer knowledge and keyboarding skills
    • Proficiency in Microsoft Word and Excel
    • Successful completion of HFHS Pharmacology test

    CERTIFICATIONS & LICENSURES REQUIRED:

    • Current licensure as a Registered Nurse by the Michigan State Board of Nursing
    • Current BLS (Basic Life Support) certification
    • Current ACLS (Advanced Cardiac Life Support) certification per HFHS Policy
