Description: This position performs the initial client home visit as well as create the Plan of Care that will be used for Assisting Hands to take care of its client on a daily/weekly basis.
Education: Licensed practical nurses (LPN), Registered Nurse (RN) licensed by the Utah Board of Nursing
Qualifications: Individual must have a minimum of 3 years of experience in the home or long-term care setting or gerontology.
Reports to: Agency Director
Primary Duties:
Visits new clients and establishes a tailored-made Plan of Care for the client.
Performs home assessments at new clients' home
May offer caregiver training on occasion
Mentors caregivers as needed.
Hours: Assignments are individual visits to the client. Expect around 10 visits a month.
FLSA Status: Non-exempt, compensation per visit with a potential monthly retainer based on availability
