    PURPOSE: The Patient Navigator guides patients through the healthcare system and works to eliminate barriers to receiving timely care and treatment across all segments of the healthcare continuum.  Assist patients and his/her family by identifying resources as they navigate through medical, insurance, financial and social issues.   Facilitate the lateral integration of the care team, collaborating with physicians, counselors, nurses, research staff and/or other staff to provide comprehensive disease management, assessment, treatment, education, and follow-up evaluation for patients.   Communicate plans of care to outside facilities.  Assist patients with scheduling, re-scheduling, follow up care, preparing for appointments and obtaining medications. Maintain appropriate tracking, data management and reporting systems for quality assurance.

    REQUIREMENTS: 

    • High School diploma or equivalent; one year of customer service or medical experience; or equivalent combination of education and experience.
    •  Familiar with guidelines regarding patient confidentiality issues.   
    • Must have working knowledge of MS Office platforms such as (Excel, OneNote, Word, PowerPoint).  
    • Ability to convey information both in writing and verbally.  
    • Must have high level of interpersonal skills to handle sensitive and confidential situations while providing exceptional customer service.
    • Independent judgment is required to plan, prioritize, and organize a diversified workload.

    PREFERENCES: Bachelor's degree in social work, psychology, public health, education or other Health & Human Services is preferred. Knowledge of technology and payer protocols desired. Experience working in a healthcare operation.  Data entry experience with basic understanding of insurance and billing procedures.  Specialized internship desirable.
