We are searching for an LPN to join our team. The LPN provides routine care, observe residents' health, may obtain vital signs, and give medication. Assists DON and communicates with residents and family members. Communicates any issues with the ADON/DON.
Essential Duties include:
• Provide direct nursing care services to Residents. Follow individualized plan of care for all Residents.
• Perform Resident assessments as required by the state standards/ regulations and community policy, under the direction DON.
• Observe Residents for change in condition and report any changes immediately to DON.. Provide timely interventions to change of condition and share this with care staff and Family as indicated.
• Document all pertinent/relevant observations, red flags, incidents, interventions, communication with the Resident’s responsible party, physician (as necessary), clinical procedures and daily medication assistance/care throughout the shift as indicated and no later than by the end of each shift. Effectively pass required information to the next shift in timely manner and as indicated to direct supervisor to ensure quality Resident care. Oversees the timely documentation by Resident assistant of services provided and red flags for their shift.
• Complete and maintain current individualized care plan for each Resident. Provide interim documentation as required to Resident file.
• Maintain the cleanliness and comfort of each resident, including bathing, feeding, and dressing
• Oversee and manage medications delivery system to include timely order entry, receipt and tracking as well as daily MAR documentation, narcotic count, and checking in medications in timely manner as assigned to ensure Resident compliance to medication regime. Notify pharmacy and supervisor of issues and seek to resolve in timely manner.
• Update resident records in a confidential manner
• Evaluate the resident’s wellbeing, reporting any needs to medical staff
• Explain treatments and procedures to residents and families
• You will be required to report ABUSE/NEGLECT in a timely manner. If you suspect abuse, neglect, misappropriation of funds, please notify the Administrator immediately.
• Maintains confidentiality of information about the facility and the individual residents.
• Assist Residents with transfers and ambulation as directed, using proper body mechanics and behavioral techniques.
• Respond to urgent calls as needed in timely manner.
• Attends mandatory in-services.
• Other duties as assigned.

Basic Qualification & Experience:
• Current licensure as a Licensed Practical Nurse in the State of Ohio
• Strong communication skills
• Good time management and organization
• Problem-solving skills
• Excellent teamwork and collaboration skills
• Extensive knowledge of basic health care procedures
• Knowledge in measuring and administering medication
• Ability to work with the elderly in a courteous and friendly manner, demonstrating patience and compassion.
