    We are expanding and looking for a compassionate, motivated Occupational Therapist to join our team. This role includes opportunities in both outpatient therapy and skilled home health, allowing you to practice across settings and support patients throughout their full continuum of care.

    Job Summary

    As an Occupational Therapist with Concept Therapy, you will work with patients from diverse backgrounds, evaluate their functional needs, and develop customized therapy plans that help them reach meaningful goals. You will collaborate closely with physicians, nurses, therapists, and caregivers to ensure coordinated follow‑up care and provide education that empowers patients to thrive.

    The ideal candidate is compassionate, organized, communicative, and comfortable managing a varied caseload across outpatient and home health environments.

    Responsibilities

    Meet with patients from a wide range of backgrounds and discuss their therapy needs with empathy and professionalism

    Conduct thorough evaluations to establish functional baselines

    Develop individualized therapy plans that support mobility, independence, and daily living skills

    Provide outpatient treatment in clinic settings and skilled therapy services in the home

    Collaborate with interdisciplinary care teams to ensure seamless follow‑up and continuity of care

    Educate patients and families on exercises, safety, adaptive strategies, and lifestyle modifications

    Maintain accurate, detailed documentation of therapy sessions and patient progress

    Qualifications

    Active Occupational Therapy license in the state of Indiana

    Strong communication and interpersonal skills

    Excellent organizational abilities and attention to detail

    Ability to manage multiple cases while maintaining a positive, compassionate approach

    Experience in outpatient, home health, or both is a plus (but not required)
