Job Overview

We are hiring a licensed Occupational Therapist (OT) for a per diem / PRN home health position. This role is ideal for clinicians seeking flexible scheduling, per-visit pay, and the opportunity to deliver patient-centered occupational therapy services in the home setting.

Key Responsibilities

Perform occupational therapy evaluations and reassessments

Develop and implement individualized treatment plans

Provide therapeutic activities, ADL training, and patient education

Document care accurately and timely in compliance with HIPAA and clinical standards

Communicate effectively with patients, families, and interdisciplinary care teams

Deliver high-quality home health occupational therapy services

Qualifications

Active Occupational Therapist (OT) license in good standing

Graduate of an accredited occupational therapy program

Ability to work independently and manage a flexible schedule

Strong clinical documentation and communication skills

Reliable transportation for in-home therapy visit

Home health experience preferred, but not required
