What You’ll Do

Conduct OASIS assessments and create individualized, outcome-driven plans of care

Evaluate patients’ physical, emotional, and environmental needs to promote independence and safety

Provide therapy and education to help patients reach their functional goals

Collaborate with physicians, nurses, and other members of the care team to coordinate ongoing treatment

Monitor progress, adjust plans as needed, and maintain accurate documentation in compliance with Medicare and agency standards

Support patients and families with compassionate, evidence-based care

What We’re Looking For

Current Florida Occupational Therapist (OT) license (required)

OASIS assessment experience (required)

Minimum of one year of home health or clinical experience preferred

Strong communication, organization, and documentation skills

Compassionate, professional, and dependable approach to patient care
