Full-Time | $75,000 – $85,000 Salary | 100% Employer-Paid Medical

Compensation

Responsibilities

• Conduct comprehensive patient evaluations in the home setting

• Develop and implement individualized treatment plans

• Address activities of daily living (ADLs) and instrumental ADLs

• Provide therapeutic interventions to improve functional independence

• Perform home safety assessments and recommend adaptive equipment

• Educate patients and caregivers on compensatory strategies

• Coordinate care with physicians and interdisciplinary team members

• Complete accurate and timely documentation

• Ensure compliance with Medicare and home health regulations

Qualifications

• Active Occupational Therapist (OTR/L) license

• Home Health experience preferred but not required

• Strong assessment and care planning skills

• Valid driver’s license and reliable transportation

• Ability to work independently in the field

• Excellent communication and organizational skills
