Occupational Therapist (OT) – Home Health
Location: Hybrid | Based in San Diego, CA
Employment Type: Full-Time
Pay Range: $40 – $60 per hour (based on experience)
A well-established and respected home health provider in the Western U.S. is seeking a dedicated Occupational Therapist (OT) to join its dynamic care team. Key Responsibilities
Conduct comprehensive evaluations of patients’ functional status, including physical, cognitive, and communication abilities.
Assess home environments to identify safety hazards or barriers to independence and recommend appropriate adaptive equipment.
Develop and implement personalized treatment plans with defined goals for functional improvement.
Support patients in regaining skills necessary for daily living or returning to work following illness or injury.
Maintain accurate and timely clinical documentation, reporting patient progress and any changes in condition to the care team and referring physician.
Collaborate with interdisciplinary team members to ensure coordinated and efficient patient care.
Provide education and training to patients’ families and caregivers on treatment protocols and safety practices.
Participate in case conferences, in-service training, and discharge planning as needed.
Qualifications
Valid and current licensure as an Occupational Therapist in the state of California.
Registration with the National Board for Certification in Occupational Therapy (NBCOT).
Minimum of two years of clinical experience as an OT; home health experience is a plus but not required.
Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
