Functional Transformation Mobile Rehab is seeking a compassionate and motivated Occupational Therapist specializing in Lymphedema Therapy to join our growing therapist-owned practice serving the Bradenton and surrounding areas.

Qualifications
Active Florida Occupational Therapy license
Certified Lymphedema Therapist (CLT) preferred; willingness to obtain certification may be considered
Strong understanding of lymphedema treatment and current therapy practices
Ability to work independently in a mobile setting while remaining connected with the clinical and administrative team
Excellent interpersonal and communication skills
Professional, dependable, and compassionate
Comfortable working with patients with a wide range of medical needs, personalities, and home environments
Commitment to continued learning and professional growth
Responsibilities
Perform comprehensive occupational therapy and lymphedema evaluations
Develop and implement individualized plans of care based on each patient's needs and goals
Provide skilled lymphedema therapy, including appropriate components of Complete Decongestive Therapy (CDT)
Assess patients' ability to safely perform activities of daily living
Educate patients, family members, and caregivers regarding treatment, self-management, and home programs
Document evaluations, treatments, progress, and outcomes accurately and in a timely manner
Monitor progress toward short- and long-term goals and adjust treatment plans as appropriate
Assist with discharge planning and recommendations for adaptive equipment when needed
Communicate with physicians, caregivers, and other members of the patient's healthcare team as appropriate
Collect and document required treatment information and follow up with patients as needed
