Licensure: An active, Licensed Vocational Nurse (LVN) license.
Experience: 1–3 years of clinical or home health administration experience.
Technical Skills: Proficiency with EMR software, medical terminology, and typing/general office tools.
Knowledge: Familiarity with HIPAA, Medicare Conditions of Participation, and Electronic Visit Verification
Order Processing: Processing, tracking, and verifying physician orders, lab results, and medication changes.
Compliance: Ensuring all patient records and documentation strictly adhere to HIPAA and home health regulatory standards.
Communication: Acting as the primary point of contact for patients, families, and doctors to relay clinical updates and schedule changes.
Office & Operations Management
Clinical Coordination & Case Management
Assist the Case Manager by coordinating clinal documentation.
Help prepare, update, and monitor patient care plans, ensuring services are delivered as prescribed.
Compliance & Documentation
Maintain accurate, up-to-date patient files and Electronic Health Records (EHR) in compliance with state and federal regulations.
Assist with clinical audits and Quality Assurance (QA) to ensure standards are met.
Monitor adherence to infection control protocols and safety standards.
Required Qualifications:
Education: High school diploma or equivalent required. A degree in healthcare administration is preferred.
Experience: Prior experience in home health, case management, or healthcare administration is preferred.
Skills:
Strong organizational and time-management skills.
Good communication skills, both verbal and written.
Ability to maintain patient confidentiality and follow HIPAA regulations.
Familiarity with medical terminology, healthcare processes, and insurance.
Certification in medical office management or case management is a plus.
Will work in an office setting
Demonstrate the ability to adapt to changing priorities and workloads in a fast-paced healthcare environment.
Support the case manager in enhancing the quality of care for patients, ensuring they receive timely and appropriate services.
Ability to work well independently and as part of a team.
