About the Role:

As an On-Call Hospice RN at Divine Guardian LLC, you will play a vital role in providing compassionate care to patients in their final stages of life. Join our dedicated team in Tucker, GA, and make a meaningful impact on families during their most challenging times.

Responsibilities:

Provide high-quality, patient-centered nursing care to hospice patients in various settings.

Conduct comprehensive assessments and develop personalized care plans.

Administer medications and manage symptom control effectively.

Collaborate with interdisciplinary teams to ensure holistic care.

Educate patients and families about end-of-life care options and resources.

Document patient care and maintain accurate medical records.

Respond promptly to patient and family needs, ensuring comfort and dignity.

Participate in on-call rotations to provide support as needed.

Requirements:

Current RN license in the state of Georgia.

Minimum of 1 year of nursing experience, preferably in hospice or palliative care.

Strong assessment, communication, and interpersonal skills.

Ability to work independently and as part of a team.

Compassionate demeanor with a focus on patient-centered care.

Basic Life Support (BLS) certification required.

Knowledge of hospice regulations and standards of care.

Flexibility to work on-call hours as needed.
