    Position Title: On‑Call LVN – Hospice (7 PM to 7 AM)

    Location: Lancaster, California Position Type: On‑Call (Night Shift) Compensation: $25 per night on-call + $75 per patient visit performed

    Position Summary

    The On‑Call LVN is responsible for providing skilled nursing care and support to hospice patients during after-hours shifts (7 PM – 7 AM). This role includes patient assessments, symptom management, and documentation of care delivered in homes or facilities. The LVN works independently during the shift and reports directly to the Director of Nursing (DON). This role is essential in ensuring patients receive compassionate, timely care during evenings, overnights, and weekends.

    Key Responsibilities

    Remain available by phone throughout the on‑call shift (7 PM to 7 AM)

    Respond promptly to patient or family calls during the on‑call hours

    Perform in-person visits as needed to assess changes in condition or provide comfort and symptom management

    Deliver direct patient care including medication administration, wound care, and other interventions per the hospice plan of care

    Provide compassionate support and education to patients, caregivers, and families during times of need or distress

    Accurately document all patient interactions, clinical findings, and care provided in accordance with agency policies

    Communicate all patient activity, concerns, or care updates to the Director of Nursing (DON) following visits

    Ensure appropriate use and handling of medications, equipment, and supplies

    Adhere to hospice policies, procedures, and state/federal regulations

    Maintain timely and accurate records for billing and care coordination purposes

    Minimum Qualifications

    Active and valid LVN license issued by the California Board of Vocational Nurse and Psychiatric Technicians

    At least one year of nursing experience; hospice or home health experience preferred

    Current BLS/CPR certification

    Valid California Driver’s License, reliable transportation, and auto insurance

    Strong clinical assessment, documentation, and communication skills

    Ability to work independently, exercise sound clinical judgment, and respond calmly in urgent or emotional situations

    Comfortable entering patient homes or facilities at night and providing care in various environments

    Compensation

    $25 per night for being on-call from 7 PM to 7 AM

    $75 per patient visit made during the on-call hours

    Mileage reimbursement provided for travel to patient locations
