Responsibilities

Perform comprehensive evaluations of patients' physical conditions, medical history, and functional abilities to develop individualized treatment plans.

Develop and implement treatment plans: Create personalized treatment plans based on assessments, incorporating various therapeutic techniques and modalities to address patients' specific needs and goals.

Provide physical therapy interventions: Administer hands-on treatments, exercises, therapeutic activities, and other interventions to improve patients' mobility, strength, coordination, and overall physical functioning.

Monitor progress and adjust treatment plans: Regularly assess patients' progress, modify treatment plans as needed, and document changes to ensure optimal outcomes.

Educate and instruct patients: Educate patients and their families on proper techniques for exercises, activities, and strategies to promote self-management and improve overall well-being.

Maintain accurate documentation: Complete necessary documentation, such as progress notes, treatment records, and other required paperwork, ensuring compliance with regulatory standards.

Collaborate with healthcare professionals: Coordinate care and communicate effectively with physicians, nurses, occupational therapists, and other healthcare team members to ensure a holistic and integrated approach to patient care.

Stay updated on industry trends: Stay informed about advancements in the field of physical therapy, evidence-based practices, and relevant regulations to enhance professional knowledge and maintain clinical excellence.

Qualifications

Candidates must possess a valid and active physical therapy license in the state where they will be providing services.

Experience in providing physical therapy services, preferably in a mobile or home healthcare setting, is highly desirable.

Candidates should have a solid foundation of clinical knowledge and skills to provide effective and evidence-based physical therapy treatments.

Clear and effective communication is essential when working with patients, their families, and other healthcare professionals. Strong interpersonal skills are necessary for building rapport and establishing trust.
