Cayaba Care is building support services to reimagine how we manage acute illness and improve outcomes in complex populations. The US is one of the most dangerous developed countries to give birth and these poor maternal-fetal outcomes disproportionately impact women of color and underserved populations across major US cities. Our team is focused on closing this gap through a combination of clinical and non-clinical services delivered in-home or in-office. Cayaba's care model focuses on integration not a replacement of patients' primary Obstetric provider and reducing any barriers to the patient receiving the recommended care. We are a multi-disciplinary team and recognize the importance of behavioral health to overall health.
WHAT YOU'LL DO
As a Cayaba Care Maternity Navigator, you will carry out in-home prenatal and postnatal visits to deliver services, you will be supported by a multidisciplinary team via telemedicine.
Be the point of contact for patients and be the clinical liaison to Cayaba Care's multidisciplinary team of nurse practitioners, nurses, social workers, and therapists.
You will play an essential role in patient outreach, patient/family engagement, delivering pregnancy health education, and assisting patients in navigating the complex healthcare systems. You will serve a role in community education and engagement.

PREFERRED CREDENTIALS
High School Diploma or equivalent required
Current national certification such as RMA, CCMA, or NCMA and CPR Certification required
2+ year experience as CMA, OB experience a plus
Doula experience a plus
Valid unrestricted drivers' license
Fluent in Spanish a plus
Experience in OBGYN setting
