    ABOUT US

    We are a Federally Qualified Health Center (FQHC) serving all those in need in the Sacramento area. We have been a level 3 Patient Centered Medical Home since 2011 and were the first to earn that designation in Sacramento. We offer integrated, comprehensive and supportive services to maximize health outcomes.     ABOUT THE POSITION

    The mission of the One Community Health Clinic is to transform lives through care, research and community awareness. This care is to be delivered with compassion and concern. Health Educators contribute to the mission by providing supportive education and coaching services to patients with regard to pregnancy prevention, management of chronic disease and prevention of communicable disease. Health Educators engage patients through on site visits, retention in care activities, community outreach events, partner counseling, referral services and targeted use of social networking.

    ESSENTIAL FUNCTIONS:

    CORE DUTIES

    • Chronic Disease Self-Management Education and Coaching
    • Communicable Disease Education, Prevention and Risk Reduction Counseling
    • Sexual Health Education and Counseling
    • Family Planning Education and Counseling
    • Community Engagement
    • Meaningful use of statistics and data
    • Interdisciplinary retention in care for individuals living with chronic disease

    ADDITIONAL DUTIES

    • Provide other administrative duties as assigned. This includes but is not limited to active monitoring of email, intranet, telephone, EMR in baskets, voice mail and person to person communication devices; scheduling, rescheduling and canceling patient's appointments as appropriate, monitoring inventory, organization, cleaning of patient care areas and storage of supplies
    • Other duties assigned based on program and organizational need

    CONTINUOUS QUALITY IMPROVEMENT DUTIES

    All staff are required to participate in quality improvement activities and actively collaborate with other clinical and non-clinical staff to ensure care is proactive and coordinated among care providers and care settings. This collaboration can include attendance in team huddles, care conferences, training, and communication. Other duties can include developing and recommending improvements to process in all areas of responsibility and participating in all other continuous quality improvement activities as appropriate.

    EDUCATION/EXPERIENCE:

    REQUIRED

    • High School Diploma, GED or equivalent
    • 1 Year Experience Customer Service Industry

    PREFERRED

    • Familiarity with chronic diseases including, but not limited to Diabetes, Hypertension, Chronic Kidney Disease, COPD
    • 1 Year of Experience in HIV/STD Education &Prevention
    • 1 Year of Experience in HIV/AIDS service
    • 1 Year of Experience in Family Planning service
    • 1 Year of Experience as care provider for Transgender and Gender Non-Conforming Individuals preferred
    • 1 Year of Experience as care provider for Sexual Minority Communities including but not limited to Lesbian, Bisexual, Pansexual, Gay and Queer identified individuals preferred
    • 1 Year of Experience as care provider to Communities of Color Including but not limited to Indigenous and Native, African American/Black, Afro-Latinx, Latinx, Asian and Pacific Islander preferred
