ABOUT US
We are a Federally Qualified Health Center (FQHC) serving all those in need in the Sacramento area. We have been a level 3 Patient Centered Medical Home since 2011 and were the first to earn that designation in Sacramento. We offer integrated, comprehensive and supportive services to maximize health outcomes. The founding values of One Community Health were based on providing a safe and welcoming place for people who often experienced social exclusion and discrimination.

ABOUT THE POSITION
The mission of the One Community Health Clinic is to transform lives through care, research and community awareness. This care is to be delivered with compassion and concern. Health Educators contribute to the mission by providing supportive education and coaching services to patients with regard to pregnancy prevention, management of chronic disease and prevention of communicable disease. Health Educators engage patients through on site visits, retention in care activities, community outreach events, partner counseling, referral services and targeted use of social networking.

All staff are required to participate in quality improvement activities and actively collaborate with other clinical and non-clinical staff to ensure care is proactive and coordinated among care providers and care settings. This collaboration can include attendance in team huddles, care conferences, training, and communication. Other duties can include developing and recommending improvements to process in all areas of responsibility and participating in all other continuous quality improvement activities as appropriate.
