    WHAT WE ARE

    We are a Federally Qualified Health Center (FQHC) serving all those in need in the Sacramento area. We have been a level 3 Patient Centered Medical Home since 2011 and were the first to earn that designation in Sacramento. We offer Integrated, comprehensive, and supportive services to maximize health outcomes.     ABOUT THE POSITION

    This position is for the successful coordination of the clinic's operational activities intended to drive organizational improvements.

    ESSENTIAL FUNCTIONS:

    Operations

    • Coordinates the completion of daily tasks, special projects and monitors clinic site workflows, processes, and utilization.
    • Contributes to the development of policies, procedures and systems which assure the effective operational functioning of clinic staff.
    • Serves as the liaison between physicians, patients and staff members. In collaboration with the Clinic Site Manager and Clinical Leadership to assist in the coordination of patient flow between departments and services.
    • Assists in coordinating patient care and access with community partners.

    Quality Assurance

    • Actively engage quality assurance activities to improve and maintain excellent customer services levels.
    • Provide feedback to staff and leadership on process improvements and data trends with actionable recommendations.
    • Focuses on continuous quality improvement of workflow processes and systems.

    Customer Service

    • Provide exceptional customer service by working with patients to meet their needs.
    • Collaborate internally across departments to identify best practices to deliver the best service to our patients.
    • Coordinates with scheduling team and leadership to address clinic coverage and access needs.
    • Coordinates resolution of patient complaints and grievances making every effort to resolve the issue in the moment

    Additional Duties

    • Coordinates with the Clinic Site Supervisor & PSR Leads to ensure coverage for the reception area and other administrative areas of the clinic.
    • Provides back up support to staff as needed.
    • Follows HIPAA guidelines and protects patients' privacy.
    • Other duties as assigned

    EDUCATION/ EXPERIENCE REQUIRED:

    • Three years previous healthcare experience.
    • Minimum of three years' experience of lead or administrative support capacity.
    • High School Diploma or equivalent required
    • AA degree in Business Administration or Health and Human Services preferred
    • Previous FQHC and/or outpatient practice management experience highly preferred.
