Position Overview
We are seeking a dynamic and experienced Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to serve as the Operations/Clinical Manager for our hospice and home health services in Cerritos, California. This pivotal role requires a blend of clinical expertise and operational acumen to lead and manage the delivery of high-quality patient care while ensuring compliance with regulatory standards.
Key Responsibilities
Clinical Leadership:
Oversee the development, implementation, and evaluation of individualized patient care plans in collaboration with physicians and the interdisciplinary team.
Ensure the delivery of patient-centered care in both hospice and home health settings, adhering to best practices and regulatory requirements.
Operational Management:
Supervise and coordinate daily operations of clinical staff, including RNs, LVNs, CNAs, and other support personnel.
Manage staffing schedules, resource allocation, and patient assignments to optimize efficiency and care delivery.
Compliance and Quality Assurance:
Ensure adherence to federal, state, and local regulations, including Medicare Conditions of Participation and accreditation standards.
Conduct regular audits and performance evaluations to monitor and improve clinical outcomes and patient satisfaction.
Staff Development and Training:
Provide ongoing education, mentorship, and support to clinical staff.
Facilitate in-service training sessions and continuing education programs to enhance staff competencies.
Collaboration and Communication:
Serve as a liaison between clinical staff, administration, and external stakeholders.
Participate in community outreach and public relations activities to promote the organization's role in the healthcare delivery system.
Qualifications
Education:
Graduate of an accredited School of Nursing.
RN: Current and unrestricted Registered Nurse license in California.
LVN: Current and unrestricted Licensed Vocational Nurse license in California.
Experience:
Minimum of 3 years of nursing experience, with at least 1 year in a hospice and/or home health setting.
Previous management or supervisory experience preferred.
Skills:
Strong leadership, organizational, and communication skills.
Proficiency in electronic medical records (EMR) systems and knowledge of OASIS documentation.
Ability to manage multiple priorities and work effectively in a fast-paced environment.
Certifications:
Current CPR certification.
Additional certifications in hospice and/or home health care are a plus.
