Orange Hospice is seeking a dedicated and experienced Per Diem RN Case Manager to join our growing team. This role is crucial in providing compassionate care to hospice patients, and we are looking for an individual with the right attitude and qualifications to excel in this position.

Responsibilities:

In charge of providing direct patient care to patients, including assessing and monitoring patients' conditions, administering medications, and performing treatments at least once a week

Collaborate with healthcare team members to develop and implement individualized care plans

Coordinate and facilitate communication between patients, families, and healthcare providers

Adhere to HIPAA regulations to protect patient privacy and confidentiality

Qualifications:

Minimum of 1 year of experience as a Registered Nurse in a hospice

Active Registered Nurse (RN) license

Must be able to drive, have a reliable vehicle, and have valid insurance

Skills:

Excellent patient service skills, with the ability to provide compassionate and empathetic care

Energetic and positive attitude

Strong critical thinking and problem-solving abilities

Effective communication skills to collaborate with interdisciplinary healthcare teams

Detail-oriented with the ability to prioritize tasks and manage time effectively

Flexible and ability to pivot based on the team's current needs
