Job Summary: We are seeking a compassionate and proactive Outreach Coordinators to support the enrollment of medical members into our Enhanced Care Management (ECM) and Community Supports (CS) programs. The ideal candidate will serve as a key point of contact for potential members, engaging them via phone calls, attending community outreach events, and providing clear and helpful information about our programs and services.
Key Responsibilities:
Conduct outreach phone calls to eligible medical members to explain ECM and CS program benefits and encourage enrollment.
Attend community health events, fairs, and partner organization functions to promote awareness of ECM and CS programs.
Build relationships with members, community leaders, and partner organizations to increase program visibility.
Educate members and their families on the value of Enhanced Care Management and Community Supports services.
Document outreach activities, member interactions, and enrollment progress accurately and in a timely manner.
Collaborate with internal teams to ensure a smooth transition from outreach to enrollment and ongoing support.
Represent the organization in a positive and professional manner at all times.
Qualifications:
High school diploma or equivalent required.
Experience in community outreach, healthcare, or member engagement strongly preferred.
Excellent communication skills, especially over the phone and in public settings.
Bilingual skills a plus.
Ability to work flexible hours, including occasional evenings or weekends for community events.
Must have reliable transportation and willingness to travel locally.
