Overview:
Serves a key role in the system-wide assessment of homeless persons to match them to the appropriate housing intervention. The Housing Assessor/Navigator will screen clients using an assessment workflow through the Homeless Management Information System (HMIS) and will provide pre-navigation services to households to obtain eligibility documents for Permanent Supportive Housing (PSH) or Rapid Rehousing (RRH). The Outreach Assessor/Navigator will also collaborate with system partners. The outreach team will also enroll and complete assessments to homeless men and women living on the street.
Duties and Responsibilities:
The Outreach Team will enroll and complete assessments to homeless men and women through the Coordinated Access System (CAS).
Provide pre-navigation services to individuals to obtain eligibility documentation for Permanent Supportive Housing and Rapid Rehousing.
The Outreach Team will provide 4 hours per week dedicated to community calls, in-reach, or encampment reconnaissance.
Connect homeless individuals to supportive services that are available through the selected providers.
Provides transportation and ongoing support to homeless individuals to and from the following activities, (apartment move-in’s, collecting documents needed for housing, obtaining IDs, Social Security Cards, Homeless Verification, Verification of Disabilities, SSI Award letter).
Transfers and upload all required homeless and housing documentation to the housing case manager.
Ensures all data is collected and entered accurately and in a timely manner into HMIS.
Attends all Housing Assessor, Housing Navigator, and HMIS trainings to fully utilize the Coordinated Access system accurately.
Provides regular feedback to management level staff of collaborating agencies regarding the Coordinated Access process.
Provides feedback to Coordinated Access Workgroup regarding Policies and Procedures of the Coordinated Access system.
Participates in case conferences as needed.
Performs other duties as needed.
Education, Licensure/Certification:
Bachelor’s degree or 3 years related experience.
Proficiency in Microsoft Office Suite.
Clear and effective written and oral communication skills.
Preferred: English/Spanish
