    PACE NORTH is seeking an Enrollment Coordinator to join our amazing team of professionals dedicated to improving the lives of older adults through an innovative, center-based program. 

    This position is responsible for activities related to participant enrollment and eligibility processes including nursing assessments and level of care determinations, as well as assisting with community education and outreach.

    The ideal candidate will be a licensed Social Worker in the state of Michigan with a valid MI driver’s license and current CPR certification.  Minimum of 2 years of experience as an Social Worker, preferably in a community based or in-home setting. 1 year of experience working with a frail or elderly population.  Ability to work effectively in a team environment.

    •  Conducts initial in-home intake assessments with potential enrollees and families to determine needs.
    •  Completes initial face-to-face Michigan Medicaid Level of Care Determination assessment with potential enrollees to determine if eligibility criteria can be met in accordance with State and Federal guidelines, as well as established PACE North policies and procedures.
    •  Maintains ongoing communication with Interdisciplinary Team about potential enrollees needs and eligibility.
    • Provides input to IDT on participants’ initial plan of care.
    • Identifies if Guardianship, Conservatorship, or Durable Power of Attorney is necessary and assists with initiation of appropriate paperwork as necessary.
    •  Works collaboratively with Community Liaison to assure timeliness in processing new enrollees.
    • Documents all activities related to initial assessment and enrollment processes as directed.
    • Regularly evaluates referrals and makes continuous improvement recommendations based on feedback.
    • Completes appropriate documentation related to participant assessments in electronic health record as required.
    • Enters Level of Care Determinations in CHAMPS as required.
    • Assists with management of referrals and referral source relationships.
    • Participates in marketing and outreach activities as directed.
    •  Maintains statistical data as required.
    • Attends IDT and other staff meetings as needed.
    • Abides by all established PACE North policies, rules, and regulations.

     

    EDUCATION: Master’s degree in Social Work

    LICENSURE/REGISTRY/ CERTIFICATION: Licensed in Michigan as Social Worker.  CPR Certification. Valid MI driver’s license.

    EXPERIENCE:  2 years of recent experience as a social worker, preferably in community-based or in-home health.  Minimum of 1 year experience working with a frail or elderly population preferred. 

    SKILLS: Exhibit excellent communication, interpersonal, and assessment skills.

    • Ability to work effectively in a team environment.
    • Efficient at multi-tasking, tracking, and organizing.
    • Possess basic computing skills in the workplace.
    • Act within the authorized scope of practice. 
    • Ability to meet a standardized set of competencies for the specific position description established by PACE North and approved by CMS before working independently.

    IMMUNIZATIONS:  Be medically cleared for communicable diseases and have recommended immunizations up to date.  All employees must demonstrate they are fully vaccinated for COVID-19 and have a current flu vaccine.
