    JOB SUMMARY: 

    Responsible for direct social work case management services to the participants of the PACE North program.  Works under the direct and indirect supervision of the Center Director or designee but is independent in the application of advanced social work knowledge and skills. Provides such social services support as participant screening, case management, counseling, and referral.                                               

    EDUCATION: Master's Degree from an accredited School of Social Work.

    LICENSURE/REGISTRY/ CERTIFICATION: LMSW or LLMSW with full licensure to be achieved within two years. Supervision for LLMSW provided on site. CPR Certification. Valid MI Driver’s License.

    EXPERIENCE:  2 years of experience in a health-related area.  Minimum of 1 year experience working with a frail or elderly population preferred.

    SKILLS: Individual therapy skills for geriatric participants.

    Analytical thinking and problem-solving abilities.

    Must be able to work effectively in a team environment.

    Good understanding of psychosocial problems of aging.

    Possess basic computing skills in the workplace.

    Act within the authorized scope of practice. 

    Ability to meet a standardized set of competencies for the specific position description established by PACE North and approved by CMS before working independently.

    IMMUNIZATIONS:  Be medically cleared for communicable diseases and immunizations are required.

    RESPONSIBILITIES:

    • Participates in the Interdisciplinary Team's initial assessments, care planning and periodic re-assessments (minimally every 6 months) of participant's case.  Assessments are to be completed prior to the scheduled team meeting. 

    • Communicates participant changes to team members.

    • Assist in development and follow-through of behavioral plans as necessary

    • Provide crisis intervention as needed, including safety planning, risk

      screenings, inpatient referral, and de-escalation

    • Provides individual and family counseling, develops, and leads group counseling and activities.

    • Refers participants and families to appropriate community agencies or facilities, acts as liaison with such organizations and as advocate for participants.

    • Encourages effective relationships among staff geared to team building and maintenance of a cohesive team.

    • Consults with and advises staff members as to the relationship of social, emotional, and cultural factors to health and medical care, and as to the availability of social services in the community.

    • Participates in Program and Policy development of Social Work component.

    • Facilitates disenrollment's from PACE North as needed.

    • Facilitates Medicaid application and redetermination process with Michigan Department of Health and Human Services (MDHHS).

    • Travels to participants’ homes to conduct site visits when needed.

    • Documents participant changes appropriately in the medical record.

    • Demonstrates active participation in QI plan processes.

    • Participates as a member of the Interdisciplinary Team.  Attends staff meetings and takes part in participant care planning. 

    • Abides by all established PACE North policies, rules, and regulations.
