SUMMARY:
The Client Care Coordinator acts as point of contact for Day and Residential clients
RESPONSIBILITIES:
● Collaborating with therapists, case managers, and clients in tracking and monitoring client’s schedules, status, location, and attendance
● Monitoring and answering emails and text messages in a timely manner ● Interacting with clients around schedule changes and updating the client schedule spreadsheet
● Coordinating with Client Services office to help communicate and distribute updates to clients
● Tracking clients’ discharge, distributing of the PCH discharge bags, tracking the completion of the discharge checklist and administering exit evaluations
● Covering the front desk for lunch, break or as needed
● Opening the center by 8:00 am (M-F)
● Opening and distributing group rosters every morning in the EHR system ● Working with lead scheduler on updating the live group schedule and the tracking of group capacity
KNOWLEDGE/SKILLS:
● Ability to manage and prioritize multiple competing tasks
● Strong sense of initiative
● Time management
● Excellent written, verbal, and interpersonal communication skills
● Highly organized and detail-oriented
● Punctual and Reliable
● Excellent knowledge of HIPAA and 42 CFR Section 164.508(b)(3)(ii) ● Knowledge of the Joint Commission/DHCS Standards is a plus
EDUCATION/TRAINING:
● High School Diploma or GED Required
● Bachelor’s Degree is preferred
