    Peak Pediatrics is a large, innovative, multi-site pediatric practice committed to the emotional and physical health of our pediatric patients. We are expanding our care coordination team and are seeking full-time Family Support Care Coordinators for our practice. Positions will take place in both our Wheat Ridge and Thornton locations and could include community and home visits. Our ideal candidate will have previous experience with pediatric social work care coordination and/or community outreach for adolescents, including child protective services.

    Job Knowledge:

    • Able to provide comprehensive social work care coordination to our families, identifying and overcoming medical, psych-social, community, cultural/religious, and financial barriers.
    • Expert knowledge of community resources and ability to build strong relationships with community partners
    • Knowledge in therapeutic higher levels of care for children and adolescents.
    • Experience with social work care coordination program development
    • Communicate and provide expertise related to social determinants of health
    • Develop and maintain strong relationships with patients and families that include assessing for care coordination needs, developing joint care plan, providing referrals, and providing education on healthcare systems
    • Expert knowledge of Medicaid and private pay insurance
    • Excellent communication and documentation skills
    • Intermediate computer and internet skills required for routine data collection and reporting; intermediate Microsoft Excel, Word, and Outlook skills, as well as experience with Epic
    • Advanced motivational interviewing skills
    • Able to work independently and as part of a multidisciplinary team

    Qualifications:

    • Masters of Social Work (MSW) or Masters of Counseling from accredited school required
    • 5 years of Social Work/Case Management experience, with at least 1 year experience in community health setting (home health, discharge planning, social services, etc...)
    • Previous pediatric case management, care plan development, and CPS experience strongly desired
    • Bilingual in English/Spanish would be ideal

    Additional Requirements:

    • Valid, unrestricted Colorado drivers license
    • Reliable and insured vehicle

    Benefits:

    • Monday-Friday 40hrs/week, with opportunity for four 10-hour days after introductory period

    Job Type: Full-time

    Salary: $60,000.00 - $80,000.00 per year

    Schedule:

    • 10 hour shift
