Job Responsibilities
Complete initial client assessments and admission visits (SOC).
Responsible for performing initial and ongoing competency assessments for Home Health Aides (HHAs) to ensure safe, compliant patient care.
Report client health status clearly and promptly to the Nurse Supervisor for proper care coordination
Travel throughout Broward County as needed
Demonstrate empathy, professionalism, and respect when working with clients and families
Ensure all documentation is complete, precise, and submitted on time.
Anticipate potential issues and appropriately escalate or resolve concerns
Prioritize tasks and demonstrate a strong sense of urgency
Required Experience & Education
Active Florida Registered Nurse (RN) license
Current Level 2 clearance in the AHCA Clearinghouse
TB test within the last 12 months or Chest X-ray within the last 5 years
Prior home care experience required - Minimum 1-2 years
Driver's License (Required)
CPR/BLS Certification (Required)
Spanish Speaking a plus!
