Core Responsibilities
Direct Client Care
Deliver personalized assistance with activities of daily living (ADL), including:
Personal hygiene (bathing, grooming, oral care)
Mobility assistance and positioning
Dressing and toileting support
Feeding and meal assistance
Medication reminders
Vital signs monitoring and documentation
Home Management
Maintain a clean, safe, and organized living environment
Prepare nutritious meals according to dietary requirements
Manage laundry and housekeeping duties
Monitor and report supply needs
Ensure home safety protocols are consistently followed
Client Support and Engagement
Provide meaningful companionship and emotional support
Facilitate social interaction and cognitive stimulation activities
Accompany clients to appointments and community activities
Assist with communication and correspondence
Support client independence while ensuring safety
Professional Responsibilities
Maintain accurate, detailed documentation of care provided
Follow individualized care plans with precision
Report changes in client condition or environment promptly
Participate in care team meetings and training sessions
Collaborate effectively with healthcare team members
Adhere to agency policies and healthcare regulations
Qualifications
Required Education and Certifications
High school diploma or equivalent
Current CPR and First Aid certification
Valid driver's license with clean driving record
Proof of vehicle insurance
Negative Tuberculosis (TB) test results

Care for non-assigned household members
Clinical procedures requiring certification
Services outside the approved care plan
