About the Role:

Argan Home Care is looking for a passionate and dedicated Physical Therapist (PT) to join our growing team in the Punta Gorda, Port Charlotte, and Englewood, FL communities. Responsibilities:

Evaluate patients' physical conditions and develop individualized treatment plans to restore function and improve mobility

Provide skilled physical therapy services in patients' homes in accordance with physician orders and plan of care

Monitor and document patient progress, adjusting treatment plans as needed to ensure optimal outcomes

Educate patients and family members on therapeutic exercises, safety techniques, and home programs

Collaborate with interdisciplinary care teams including physicians, nurses, and other healthcare professionals

Maintain accurate and timely clinical documentation in compliance with regulatory and agency standards

Participate in discharge planning and coordinate continuity of care for patients transitioning between levels of care

Requirements:

Valid Florida Physical Therapist (PT) license in good standing

Graduate of an accredited Physical Therapy program (Master's or Doctorate preferred)

Home health or outpatient experience preferred; new graduates encouraged to apply

Strong knowledge of physical rehabilitation, therapeutic exercise, and manual therapy techniques

Excellent communication, time management, and organizational skills

Reliable transportation and valid driver's license for home visit travel throughout Port Charlotte, FL

CPR/BLS certification required or ability to obtain upon hire
