  • Responsibilities

     

    POSITION SUMMARY

     

    Assists in coordination of in-home support services to meet the needs of Hospice patients and their families.  Receives and handles daily and after-hours calls (night, weekend, and holidays) from hospice patients and their families. Performs after-hours introductory visits and admissions. Will be responsible for providing nursing services consistent with high standards of performance and conduct with currently available knowledge and nursing skills. Applies diagnostic and crisis intervention skills. Coordinates resources available to meet patient and the family’s needs. Reviews all processes on a yearly basis to assure those processes are current and within or exceeding guidelines. Assists with and completes other tasks as requested.

     

    • Completes introductory visits to explain hospice services to patients and families.
    • Completes admission process for patients to hospice as needed, 24/7.
    • Readily make home/facility visits to the patient to assess status and support caregivers.
    • Assess and evaluate patient’s condition and develop plan of care based on assessment.
    • Performs prescribed medical treatment, including pain management and symptom control, under direction of primary physician or Hospice Manager.

    Required Skills

     

    • Bachelor’s degree in Nursing preferred or diploma with
    • 3 Years experience working with elderly or terminally ill.
    • Current CPR certification.
    • Ability to move independently or with reasonable accommodation within the facility and community.
    • Applicant must be physically and mentally able to perform all requirements of the job.
    • Applicant must pass background and DMV check.

    Required Experience

     

    • Licensed in the State of Montana, good standing.
    • Must have knowledge of the application of diagnostic and crisis intervention skills.
    • Must have the ability to respond calmly and effectively in stressful situations; assess patient needs; reassess continuing patient needs; communicate effectively with other health care providers.
    • Strong, effective written and verbal communication and interpersonal skills.
    • Must possess strong organizational and time management skills.
