    The SENIOR FORMS DESIGNER/SYSTEM ADMINISTRATOR is responsible for overall management and support of the forms software application, including implementation, testing and maintenance.  Provides technical support to the Help Desk and assistance in resolving/trouble shooting all end user issues upon request. Drives forms standardization and ensures regulatory requirements governing medical record documentation are adhered to.  Maintains the inventory of document types with their assigned barcodes.  Is a liaison between department managers, Design Team, and Forms Committee to ensure that appropriate form changes are made timely and accurately. Serves as forms specialists for PIH Health Physicians and PIH Health Hospital Downey so that consistency across the Integrated Delivery System is achieved.

    Required Skills

    • Microsoft Office products experience. 
    • Understanding of windows server functionality including file and directory structures, mapping drives, and network access.  
    • Needs to be able to identify and provide complete descriptions of datastreams to be merged with client forms.  
    • Must possess strong verbal and written communication skills, and have the ability to perform multiple tasks with frequent interruptions. 
    • Must have good trouble shooting skills. 
    • Must have a general understanding of HL7 and importing of files.  
    • Must have strong organizational skills, analytical skills and work within scope of projects by meeting commitments and timelines.  
    • Must exhibit strong customer service skills and possess the ability to work well with people from different disciplines/departments.  
    • Must be detail oriented. 
    • Ability to comprehend workflow and integration of the forms application within the current hybrid environment. 
    • Must have strong trouble shooting and communication skills to work with Information Technology staff, end users and the vendor.

    Required Experience

    • Must have a High School diploma or G.E.D.  
    • Previous Healthcare experience is required.
    • Forms design experience required.  
    • Work experience with systems and applications as well as knowledge of use of Adobe and conversion of PDF and TIF files preferred.
    • Knowledge of medical record documentation requirements (The Joint Commission, Title 22) preferred.
    • RHIT/RHIA or equivalent on the job experience is required.
