Our client is seeking a compassionate Hospice Medical Social Worker with a Master’s in Social Work and relevant experience to join their care team for a PRN opportunity. The role involves collaborating to provide patient care plans and supporting patients in the Traverse City, MI.

Our client is a home health and hospice company with patients throughout the state of Michigan.

Requirements:

• Limited License MSW, a minimum of 1 year of social work, hospice experience preferred

Details:

• Pay Rate/Salary: $28-30/hr

• Schedule: TBD – most likely 4 shifts per month, with 2 shifts being on weekends

• Territory/Driving Expectation: Up to 1 – 1.5 hours, Wexford, Grand Traverse Counties
