POSITION SUMMARY The PRN LPN is responsible for conducting initial nursing assessments as needed and completing reassessments as required to ensure clients receive appropriate and compliant care. This role requires strong clinical judgment, thorough documentation skills, and the ability to work independently in the field. Responsibilities ● Conduct initial nursing assessments as assigned ● Perform client reassessments as needed ● Accurately complete all required assessment forms and clinical documentation ● Submit completed documentation to the office in a timely manner ● Communicate effectively with office staff regarding client status and care needs ● Ensure compliance with Virginia home care regulations and agency policies ● Travel to client homes within assigned service areas Qualifications ● Current, active Virginia LPN license in good standing ● Minimum 1 year of clinical nursing experience preferred ● Home care experience preferred ● Experience with WellSky (or similar EMR system) is a plus ● Strong documentation and organizational skills ● Reliable transportation and valid driver’s license ● Willingness to travel to Virginia Beach, Norfolk, Chesapeake, Suffolk, and Portsmouth
