    Physical Therapist – Per Diem
    Location: Boynton Beach, FL

    Position Summary

    We are seeking a dedicated and experienced Per Diem Physical Therapist to join our home health team. The ideal candidate will have a strong background in delivering skilled therapy in the home setting and must have experience treating patients with lymphedema. This role is essential in coordinating and delivering high-quality, cost-effective care in a manner that enhances patient outcomes and satisfaction.

    Essential Responsibilities

    • Provide physical therapy services in accordance with a physician’s written plan of care

    • Conduct comprehensive assessments of patients’ rehabilitative and functional needs, including home environment evaluations

    • Evaluate mobility, strength, gait, and range of motion to develop tailored care plans

    • Direct and administer therapeutic interventions and physical therapy treatments

    • Instruct patients, families, and caregivers on the safe use and care of therapeutic equipment

    • Monitor patient progress and response to treatment; communicate changes to the physician

    • Initiate and modify therapy programs; guide caregivers and staff on appropriate support techniques

    • Participate in care planning meetings and case conferences

    • Train patients in the use of prosthetic devices as needed

    • Identify needs for additional home health services and make appropriate referrals

    • Document clinical visits, treatment plans, and progress summaries per policy and best practices

    • Provide consultation and education to families/caregivers and participate in discharge planning

    • Offer in-service education to internal clinical teams as needed

    • Supervise Physical Therapy Assistants in compliance with state and agency standards

    • When therapy is the sole skilled service, supervise and evaluate home health aide services every two weeks

    • Adhere to all HIPAA and confidentiality regulations regarding protected health information (PHI)

    Qualifications

    • Active Physical Therapist license in the state of Florida

    • Minimum of one year of experience in home health or outpatient setting

    • Demonstrated experience treating patients with lymphedema is required

    • Strong clinical assessment, documentation, and communication skills

    • Ability to work independently and as part of an interdisciplinary team

    • Reliable transportation and willingness to travel locally for home visits
