Physical Therapist – Per Diem
Location: Boynton Beach, FL
Position Summary
We are seeking a dedicated and experienced Per Diem Physical Therapist to join our home health team. The ideal candidate will have a strong background in delivering skilled therapy in the home setting and must have experience treating patients with lymphedema. This role is essential in coordinating and delivering high-quality, cost-effective care in a manner that enhances patient outcomes and satisfaction.
Essential Responsibilities
Provide physical therapy services in accordance with a physician’s written plan of care
Conduct comprehensive assessments of patients’ rehabilitative and functional needs, including home environment evaluations
Evaluate mobility, strength, gait, and range of motion to develop tailored care plans
Direct and administer therapeutic interventions and physical therapy treatments
Instruct patients, families, and caregivers on the safe use and care of therapeutic equipment
Monitor patient progress and response to treatment; communicate changes to the physician
Initiate and modify therapy programs; guide caregivers and staff on appropriate support techniques
Participate in care planning meetings and case conferences
Train patients in the use of prosthetic devices as needed
Identify needs for additional home health services and make appropriate referrals
Document clinical visits, treatment plans, and progress summaries per policy and best practices
Provide consultation and education to families/caregivers and participate in discharge planning
Offer in-service education to internal clinical teams as needed
Supervise Physical Therapy Assistants in compliance with state and agency standards
When therapy is the sole skilled service, supervise and evaluate home health aide services every two weeks
Adhere to all HIPAA and confidentiality regulations regarding protected health information (PHI)
Qualifications
Active Physical Therapist license in the state of Florida
Minimum of one year of experience in home health or outpatient setting
Demonstrated experience treating patients with lymphedema is required
Strong clinical assessment, documentation, and communication skills
Ability to work independently and as part of an interdisciplinary team
Reliable transportation and willingness to travel locally for home visits
