Our SFC Case Managers play a vital role in ensuring that every family we serve is assisted in navigating the VA Waiver Program and resources.

Your Impact:

Empower families on their journey to self-sufficiency by providing critical support as a Temporary SFC Case Manager, all while working closely with the Data Clerk. Be a hero for families in need by ensuring they receive uninterrupted services through efficient home visits and document processing. Proactively visit clients in their homes, review essential documents, and gather necessary signatures to keep their services running smoothly.

Flexible Scheduling: With our new Temporary SFC Case Manager position you can work full-time or part-time hours, while also setting your own schedule. Note: You MUST have availability during regular business hours to do the required initial training.

Experience and Education: Minimum of an Associate's Degree in Human Services or similar field with 2 years of relevant experience; or a Bachelor's Degree in any field and 3 years of relevant experience.

Availability: You are in control of your own availability! Work full time or part time hours! Create your own schedule! Note: You must be available during regular business hours for the initial training.

Bonus Points:

Prior experience with Medicaid waivers.

Training in Person-Centered Thinking/Planning.
