Ascend Mental Health is seeking a passionate and engaged Rehabilitation Specialist who thrives on excitement and quality service delivery to clients. The Rehabilitation Specialist oversees the psychiatric rehabilitation program and ensures that the service provided by our Direct Care Counselors reflect the need of the clients served by the program. The Rehabilitation Specialist is responsible for providing oversight for the daily program rehabilitation services, and collaborates with our Program Directors.
Duties:
Provide clinical oversight and guidance to Direct Care Counselors serving youth clients in the PRP program
Develop, review, and approve Individual Rehabilitation Plans (IRPs) with measurable goals and interventions
Review and approve progress notes, monthly reports, and documentation for quality, accuracy, and compliance with COMAR and Medicaid standards
Conduct initial and periodic case reviews with staff, clients, and families to track progress and adjust services
Collaborate with therapists, case managers, school personnel, and community partners for coordinated care
Lead staff supervision, training, and performance development for PRP team members
Monitor and audit program documentation for quality assurance, addressing any deficiencies promptly
Engage in outreach, community education, and relationship building to promote the PRP program and increase referrals
Identify and pursue opportunities to grow the youth PRP caseload in alignment with agency mission and vision
Attend required meetings, maintain timely documentation, and uphold all HIPAA, safety, and professional standards
Maintain two in-office days per week to support supervision, program development, and collaboration
Qualifications: Must be a licensed mental health professional with a minimum of 2 experience working with emotionally disturbed youth OR be a Certified Psychiatric Rehabilitation Practitioner (CPRP) in good standing.
