**Job Description:

** The primary function of a Registered Nurse – Emergency Room is to provide chronic care management to high-risk patients, perform triage (phone and in-person), and assist in caring for chronic and primary care patients.

Requirements:

• Valid registered nursing license in the State of California
• Basic Life Support (BLS) Certification
• Advanced Cardiovascular Life Support (ACLS) Certification
• Pediatric Advanced Life Support (PALS) Certification
• National Institutes of Health Stroke Scale (NIHSS) or Stroke Certification
• Minimum of 1 year ER RN experience

**Key Responsibilities:

** **Chronic Care Management:

** Conduct comprehensive education for all chronic illness patients, with a focus on diabetes, hypertension, asthma, hyperlipidemia, and ischemic vascular disease

**Patient Care:

** Triage patients via phone and in-person, including walk-insProvide patient education on dietary/lifestyle modifications, medication teaching (DM, HTN, HL, asthma), glucometer usage, asthma education, and preventative health screeningsConduct spirometry testing for asthma and other patients as ordered by NP

**Management and Supervision:

** Supervision of students in Medical Assistants and Nursing student interns/externs
Oversees the Vaccine inventory/ordering: VFC, 317 and insured.
Keeps up-to-date CDC VIS sheets. Duties may be assigned to Medical Assistants, but oversight is the responsibility of the RN

Salary:**

$ 649.04 per day
