About the Role:We are seeking a skilled and compassionate Physical Therapist (PT) to join our Home Health team. The ideal candidate will provide high-quality rehabilitative care to patients in the comfort of their homes. This role offers flexibility, competitive compensation, and mileage reimbursement.
Key Responsibilities:- Conduct initial patient evaluations and develop individualized treatment plans
- Provide hands-on therapy to improve mobility, strength, and overall function
- Educate patients and caregivers on exercises and techniques to support recovery
- Maintain accurate and timely documentation in accordance with home health regulations
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes
- Manage a caseload of 30 points per week
Qualifications:- Licensed Physical Therapist (PT) in the state of Florida
- Home Health experience preferred
- Proficiency with EMR systems (HomeCare HomeBase, Kinnser WellSky, or similar)
- Strong communication and organizational skills
- Ability to work independently and travel between patient homes
