If the facility moves forward on submission, additional questions will be addressed during the phone interview. All providers are expected to float to COVID Units. Full PPEs will be provided. The following documentation items shall be submitted prior to Supplier Employee assignment:
National Background Screen, including the OIG Database
Drug Screen (at least gone to clinic – pending results)
TB within 12 months
License and applicable certificate’s
All additional required documentation shall be submitted no later than 7 business days from assignment start date
