We are currently seeking a skilled and compassionate Hospice Medical Social Worker to join our team.

Job Description:

We are looking for a dynamic and empathetic Hospice Medical Social Worker to provide comprehensive psychosocial support to our patients and their families. The ideal candidate will have a strong background in medical social work, a deep understanding of end-of-life care, and exceptional communication skills. The Hospice Medical Social Worker will be responsible for assessing the psychosocial needs of patients and families, providing counseling and support, coordinating community resources, and advocating for the holistic well-being of the patients. This role will collaborate closely with interdisciplinary teams to ensure the highest quality of care and support for our patients and their families.

Responsibilities:

• Conduct comprehensive psychosocial assessments of patients and their families to identify their specific needs and concerns.
• Provide counseling, emotional support, and crisis intervention to patients and families coping with end-of-life issues.
• Collaborate with interdisciplinary teams to develop and implement effective care plans that address the psychosocial needs of patients and families.
• Coordinate community resources and referrals to ensure access to appropriate support services, including financial assistance, bereavement counseling, and other necessary resources.
• Advocate for the rights and preferences of patients and families, ensuring their voices are heard and respected within the care team and the community.
• Maintain accurate and timely documentation of all patient interactions and interventions in compliance with organizational standards and regulatory requirements.
• Participate in ongoing education and training to stay updated on best practices in hospice care and medical social work.

Qualifications:

• Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent certification.
• Minimum of two years of experience in medical social work, preferably in a hospice or palliative care setting.
• Strong understanding of the physical, emotional, and spiritual challenges faced by patients and families at end of life.
• Excellent communication, interpersonal, and counseling skills.
• Ability to work collaboratively in an interdisciplinary team and adapt to changing environments.
• Empathy, compassion, and a commitment to providing holistic care to patients and their families.
