    Job Title: Family Case Manager FLSA Status: Non-Exempt

    Department: School & Community Youth Services

    The Family Case Manager provides the following services in Montgomery County, MD.

    Full Time, Days and Evenings, Monday through Friday 8am to 4pm some evenings might be required to meet with families/students

    General Summary:

    Provide culturally and linguistically appropriate and comprehensive case management services for the School and Community Youth Services (SCYS) school-based mental health and social services program offered in collaboration with Montgomery County Public Schools (MCPS) and the Department of Health and Human Services (DHHS).

    The services are to promote self-sufficiency skills, family development, and linkage to mental health resources which are targeted to un- and under-insured youth and their families, newly arrived families, or students/families identified by the school as needing assistance in accessing resources.

    Experience facilitating groups to support the services listed above.

    Collaborate with the therapist, school staff, and community partners in providing comprehensive care while maintaining strict confidentiality of client information.

    Principal Responsibilities and Duties:

    Provide support groups and case management services to students/parents/families with economic and/or social needs such as housing, healthcare, employment assistance, food, clothing, education, and other services.

    Use the County’s electronic records system to document service plans and update plans every six months. Maintain regular contact (at least twice a month) with clients.

    Data must be entered into the system within one business day of the date that the services were delivered.

    Maintain a caseload in compliance with contract terms and agency expectations as defined by the program director.

    · Organizes, coordinates, and facilitates a wide variety of meetings, workshops, presentations, activities, and events to develop partnerships between families and schools.

    · Work closely with service providers and develop a referral system to promote a continuum of care.

    · May conduct home visit meetings as necessary and/or make referrals, as necessary.

    · Participates in workshops and conferences as requested by immediate supervisor.

    Knowledge, Skills, and Abilities Required:

    Bachelor’s degree in Social Work (B.S.W.), Psychology, or related field. Prior experience serving parents/families in need (internship experience considered). Prior experience can also be substituted for each year of required education.

    Experience working within collaborative school-based models of service delivery is preferred.

    Prior experience/equivalent knowledge of immigrant populations & acculturation issues preferred.

    Knowledge of community resources in the Montgomery County area preferred.

    Generally, must possess a valid state driver's license with 3 or fewer points at time of hire.

    Bilingual preferred (English/Spanish) but not necessary.
