    CNA- CERTIFIED NURSING ASSISTANT

    As a CNA, you will have the unique opportunity to positively impact our residents every day. You will partner with a multidisciplinary team of healthcare staff, and under the supervision of a registered nurse (RN), positively represent the facility, providing routine patient care to promote patient comfort and contribute to the overall patient plan of care.

    This position is responsible for on-going patient data collection and documentation and providing a variety of unit support functions which contribute to the overall efficient operation of the nursing unit/department.

    As a CNA you will,

    • Provide assistance in activities of daily living and complete necessary tasks to promote patient health, including assisting patients with basic hygiene needs, maintaining an optimum level of mobility and assisting patients with nutritional intake, as needed
    • Collect patient data, including vital signs, input/output and other delegated measurements in accordance with unit/facility policy and standards of care
    • Document objective data and routine aspects of patient care
    • Recognize and report abnormalities and/or changes in the patient’s health status and responses to treatment to the licensed nurse
    • Promote continuity of care by accurately and completely communicating to other caregivers the status of patients for which care is provided
    • Perform procedures and use equipment as delegated by a licensed professional, consistent with demonstrated competencies and state scope of practice
    • Provide necessary supplies, support and assistance to medical staff and patients for unit specific procedures, under the direct supervision of a licensed professional
    • May provide general support to the department, including greeting and assisting patients, visitors, physicians and ancillary personnel

    . QUALIFICATIONS:

    • As a CNA, you will need a high school diploma or equivalent and must be a CNA in good standing
    • 1 year of experience as a Certified Nursing Assistant (CNA), experience in a long-term care facility preferred
    • Ability to stand for long periods of time, lift and transfer residents (up to 100 pounds unassisted), bend, twist, push and pull
    • Knowledge of medical terminology, skill in nurse aide techniques
    • Exceptional organization skills, ability to prepare and maintain accurate resident charts, reports and observations
    • Effective communication skills and interpersonal relations
    • Good problem solving and independent thinking skills
    • Knowledge of basic hygiene and sanitation

    Are you a dedicated Registered or Licensed Vocational Nurse passionate about long term care and looking for a progressive career in healthcare? Do you want to work with a LEADER IN THE REHABILITATION arena, SPECIALIZING IN LONG TERM CARE? As a Charge Nurse you will assess resident needs, develop individual care plans, administer nursing care, evaluate nursing care and supervise Certified Nursing Assistants (CNAs) in the delivery of nursing care. In this role, you will be involved in hiring, orientation and monitoring the nursing staff and training them to do their best while working closely with doctors, administering medications and maintaining accurate documentation of treatment.

    JOB RESPONSIBILITIES

    As a RN/ LVN you will supervise the nursing unit and coordinate the effective use of personnel, supplies, equipment and facilities. Additional responsibilities include:

    • Reviewing staffing patterns and census of nursing units; reassigning personnel when necessary to ensure residents needs and budgetary controls are met
    • Managing and analyzing patient care following admission/readmission; conducting initial assessment for newly admitted or readmitted patients
    • Assisting in creating and implementing care plans that address individual residents' needs
    • Administering medications, IV therapy, gastronomy tube management as well as some wound care
    • Monitoring residents and communicating changes to physicians
    • Ensuring appropriate documentation of resident care and maintaining medical records to ensure safe, efficient and therapeutic care to residents
    • Troubleshooting and investigating complaints, taking necessary action to resolve problems as necessary.

    ADDITIONALLY, YOU MUST HAVE:

    • RN or LVN License, in good standing
    • 1 year of experience as a nurse, experience in a long-term care facility preferred
    • Effective communication skills
    • Ability to organize, prioritize and maintain confidentiality and react decisively in emergencies
    • Exceptional supervisory skills, taking a proactive role to supervising and developing nursing team
    • Excellent training skills, ability to provide hands-on training to nursing staff
    • Good problem solving and independent thinking skills
    • Case management skills, comfort with client relations
