General Description:

The Substance Use Housing Care Coordinator is a dedicated professional tasked with providing community-based support to individuals with Substance Use Disorder who are either experiencing homelessness or have recently been housed through the Brevard Homeless Coalitions' Coordinated Entry System. The position is focused on reducing behavioral barriers to obtaining and maintaining stable housing. The Housing Coordinator will engage in:

Key Responsibilities:

Outreach services in various environments including but not limited to community common areas, woods, shelters, and residential treatment facilities.

Behavioral health needs assessments, encompassing medical, social, and housing needs.

Establishing linkages to a broad range of community resources.

Facilitating transitions between states of homelessness to housing stability and recovery.

Developing and monitoring individualized care plans.

Building partnerships and collaborations with community resources.

Requirements:

Formal Education:

Bachelor's Degree in psychology, social work, counseling, or related human services field is preferred.

Work Experience:

At least two years of experience providing community-based treatment services.

Experience working with individuals with substance use disorders, particularly those facing homelessness or housing instability.

Required Licenses, Certifications, Registrations:

Valid driver's license required.
