General Description:

The SOAR Specialist plays a pivotal role within the Space Coast Health Center ER Diversion Program and Related Programs, addressing the multifaceted needs of individuals presenting with behavioral health concerns. This specialist's primary focus is to facilitate access to Social Security disability benefits and entitlement programs, thereby fostering resiliency to reduce emergency department dependency. The SOAR Specialist's integration with the existing hospital framework magnifies the ER Diversion program’s mission – minimizing unnecessary admissions, optimizing resource allocation, and enhancing behavioral health outcomes.

Key Responsibilities:

Conducting eligibility screenings for SSI/SSDI and other entitlements.

Assisting with the SOAR application process and compiling necessary documents.

Advocating and providing persistent support through the benefits acquisition phase.

Collaborating with the care coordination team to intertwine benefits seamlessly into holistic care plans.

Engaging in community outreach efforts to uncover potential program participants.

Documenting interventions and progress in alignment with hospital protocols and program goals.

Requirements:

Formal Education:

Bachelor’s Degree from an accredited university in human services or related field, encompassing human behavior studies and advocacy techniques.

Work Experience:

Minimum of two years of direct experience in a role with similar capacities, specifically oriented towards entitlement program navigation. Experience in behavioral health contexts is highly advantageous.

Required Licenses, Certifications, Registrations:

Valid driver's license required.
