Silver Care is seeking a compassionate, dependable, and experienced Part-Time Home Care Nurse to join our team!

This position is ideal for a nurse looking for a flexible part-time opportunity providing in-home assessments, supervisory visits, care planning, and caregiver support throughout the Hampton Roads area.

Position: Part-Time Home Care Nurse

Location: Hampton Roads, Virginia Norfolk • Virginia Beach • Chesapeake • Portsmouth • Suffolk • Hampton • Newport News • Yorktown • Williamsburg • Smithfield • Windsor and surrounding areas

Responsibilities

Conduct initial client assessments and ongoing supervisory visits

Assess client needs and develop or update individualized care plans

Complete required client documentation and paperwork accurately

Communicate with Silver Care staff and community partners regarding client needs and service authorizations

Monitor client progress and changes in care needs

Provide guidance and training to Personal Care Aides based on client care plans

Observe and verify Personal Care Aide skills and competencies as required

Assist with personal care needs when necessary

Travel to clients' homes throughout the designated service area

Qualifications

Valid nursing license in the Commonwealth of Virginia

Home care or similar healthcare experience preferred

Strong assessment, communication, and documentation skills

Compassionate and professional approach to client care

Ability to work independently and manage a flexible schedule

Comfortable utilizing electronic documentation systems such as AxisCare

Valid driver's license and reliable transportation

Ability and willingness to travel to client homes throughout the service area

Comfortable visiting homes that may have pets or smoking
